Ask people something. Read what they said.
A form is built from blocks: a written answer, a multiple choice, a date, or a note that just tells people something. Attach it to the project it is about, share it inside the workspace or as a link anyone can open without an account, and the answers come back where the work is.

Two audiences that never meet.
The person who builds the form is inside the workspace. The person who answers it often is not, and has no account and never will. Everything here follows from that.
Built from blocks, in any order
A written answer, a multiple choice, a date, or a note that asks nothing and just tells people something. Any block can carry a picture or a link. Drag to reorder; a choice block's options are the same object the product uses everywhere else it asks somebody to pick.

A picture or a link sits on any of them.
A link anyone can open, with no account
Turn the link on and anybody who has it can answer. They see the questions and nothing else: not your workspace's name, not the project, not the creator, not how many people have already replied. The page is never indexed by search engines.

Anonymous means the column is not written
Choose it when you make the form and nothing about who answered is recorded, for anybody, including your own members. It is fixed at creation and cannot be switched afterwards in either direction, so what somebody was told when they answered stays true. You still see how many opened it and how many replied.
The numbers, then the answers
Opening your own form shows the overall picture: how many replied, how the choices split, the span the answers cover. A second tab has every individual response in full, and the whole thing downloads as CSV.
It belongs to a project
Attach a form to a project and it appears on that project's own Forms tab, so the answers sit beside the work they are about rather than in a separate tool somebody has to remember.
Collaborators, and a record of who changed what
Add colleagues who can edit the form with you. A panel shows who it was shared with, who has opened it and who has answered, and every change to the form is logged with the person who made it.
